1st Annual Woodland Elementary PTO Family Fun 5K Run/Walk

I knowingly, and at my own risk, I am participating in the Woodland Elementary School 5K Run, Walk and/or Fun Run. I do hereby waive release any and all claims against the Woodland Elementary School PTO, Southwick/Tolland/Granville Regional School District, all event sponsors and any employee, volunteer, or officials of these organizations from any claim of injury (including death) that I may incur as a result of my participation in the event. I further hereby certify that I have full knowledge of the risks involved in this event, and I am physically fit and sufficiently trained to participate. If, however, as a result of my participation in the Woodland Elementary School PTO 5K Run, Walk and/or Fun Run, I require medical attention, I hereby give consent to authorize medical personnel to provide such medical care as deemed necessary. The Woodland Elementary School PTO reserves the right to cancel this, or any portion of this event, at any time. No refunds will be granted under any circumstances, including event cancellation and by signing this I acknowledge said policy. I, as a participant, will be responsible for all fees that will be incurred should I change/modify my registration or change my registration to another eligible participant. All registration deadlines are firm with no exceptions. This includes any and all deadlines related to event t-shirts. Also, for the safety and comfort of all, NO DOGS will be allowed at the event. For the safety of all, only registered participants will be allowed on route. WES PTO reserves the right to modify/alter the race routes, at any time, to meet logistical requirements of the events. ***IF UNDER THE AGE OF 18, PARENT/GUARDIAN IS REQUIRED TO SIGN THIS WAIVER*** SecureFee™ Registration Cancellation Insurance is offered in most states.
