The Run For A Child

STEVEN�S HOPE FOR CHILDREN WAIVER/RELEASE EVENT: The Run For a Child September 6, 2014 (Note: Participants, volunteers, and support crew must sign this waiver/release) In consideration of the acceptance of my entry into the Steven�s Hope for Children �The Run For a Child� 5K & 10K 1 Mile Fun Run, and with full knowledge and appreciation that running is a hazardous activity, I hereby waive, release and forever discharge any and all rights and claims and damages which I may have or which may hereafter accrue to me against Steven�s Hope for Children, the City of Chino, Prado Regional Park, and the county of San Bernardino, all municipalities and special districts and properties to which this event will pass, start or finish, its volunteers, and all of their sponsors and promoters of this event, including the Director, officers and board members, the promoting clubs and organizations, officials, or other respective officers, agents, representatives, successors, and/or assigns for any and all damages which may be sustained or suffered by me in connection with said association and/or damages arriving out of my traveling to, participation, or returning from this event. I agree to abide by the rules and regulations of this event, Steven�s Hope for Children, and the vehicle laws of the State of California. I certify that I am physically fit, have sufficiently trained and prepared for participation in this event, and have not been advised otherwise by a qualified medical person. I acknowledge that this athletic event is a test of a person�s physical and mental limits and carries with it the potential for death, serious injury and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of athletes, lack of hydration, equipment, vehicular traffic, actions of other people including, but not limited to participants, volunteers, spectators, journalists, event officials, event monitors, and/or producers of the event. The risks are not only inherent to athletes, but are also present for volunteers and support staff. I hereby assume all of the risks of participating and/or volunteering in this event called Steven�s Hope for Children �The Run For A Child�. I personally accept responsibility for any injury or property damage to myself or to any other person(s) or property due to my failure to comply with all traffic rules and regulations, or event regulations and rules. I hereby consent to receive medical treatment at my own expense (including ambulance service), which may be deemed advisable in the event of injury, accident, and/or illness during this event. I have read, understand, and agree to abide by the rules of the event as published on the event website. I hereby give the Steven�s Hope for Children, the right and permission to copyright and/or use, publish, and reuse and republish photographic pictures or portraits made of me without compensation, for the purpose of advertising and promotion of Steven�s Hope for Children at this event or any activities related to this event. I hereby release and discharge the agency, its clientele, the photographer, representatives, assigns, employees, or any person or persons, corporation or corporations, acting under the permission of the agency, including any firm publishing or distributing the finished product relating to claims I might have in the advertising or any photographs used. I hereby waive any right to approve the finished photograph or any copy, which might be used in conjunction with the finished photograph. I hereby warrant that I am over 18 years of age, and am competent to contract in my own name, insofar as the above is concerned. I acknowledge that this accident waiver and release of liability form will be used by the event holders, sponsors, and organizers, in which I may participate and that it will govern my actions and responsibilities at said events. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assigns. This accident waiver and release of liability form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I understand that if I participate in any raffles, auctions, giveaways or contests while at this event, I am solely responsible for payment of any licenses, taxes, delivery fees, etc. I certify that I have read this document and I understand its content.
