I have enrolled in and/or am participating in an event (the “Event”) offered by RUN 2 BE FIT/PNW LADIES RUNNING GROUP. The Event can include, but not limited to: run/walk (race), running training, hiking, group/personal training, group run/walks.
I will consult with my physician, assess my fitness level, & follow all safety instructions before beginning this event. I understand that I should not participate in the event if I have any physical or health limitations. The Event can be a physically demanding event that requires use of common sense and self-awareness to avoid injury.
I know that running is a potentially hazardous activity and certify that I am in good health and physically fit to enter into a training program. I acknowledge that I am aware of the may risks involved in athletic training in general and running training specifically, which risks include by way of example and not limitation: 1) minor injuries such as scrapes, bruises, sprains, and strains, 2) more serious injuries such as joint, muscle and bone injuries, concussions, and other head injuries, heat-related injuries, such as heat stroke and heat exhaustion, dehydration and over-hydration conditions such as hyponatremia, and catastrophic injures and conditions such as heart attacks and other conditions or injuries which could be fatal.
I fully understand that there are certain risks of physical injury in connection with the Event, intending to be legally bound hereby, I, the undersigned, for myself as well as my heirs, and legal representatives, expressly agree to:
1) Release, waive, and discharge Arnie & Grace Martinez, Run 2 Be Fit, PNW Ladies Running Group, PNW Ladies Running Group Ambassadors, and any of each of their respective representatives and successors from all manner of actions and causes of actions, suits, debts, accounts, judgments, claims, and demands whatsoever in law or equity (including costs and attorney’s fees), including all claims arising out of or related to any incidents involving personal injury, arising in any way by participation in the Event.
2) Not commence the Event unless I know I am in the proper physical condition (including by obtaining proper advice from my medical provider if applicable) and immediately stop any exercise or use of the program should I feel pain, dizziness or any other discomfort.
3) Assume any and all risks involved in or arising from my voluntary participation in the Event, including without limitation, the risks of death, bodily injury, or property damage.
I give permission to have my photo/video taken during events, etc. for publicity purposes by Run 2 Be Fit/PNW Ladies Running Group. If I have a grievance, I must give Run 2 Be Fit/PNW Ladies Running Group the opportunity to resolve issue in lieu of making bad reviews that negatively impact Run 2 Be Fit/PNW Ladies Running Group. My signature below indicates that I have read all of the above material and fully understand this waiver as well as the risks and hazards that apply to participation in the Event. By signing this document I realize that I am waiving certain legal rights, and I have done so voluntarily.
Member Code of Conduct policy:
· Always show respect your fellow club members at all times;
· Always show respect and appreciation for the volunteers who give their time to help the club and/or event(s);
· Never yell, taunt, or threaten physical violence upon another member of the club, a volunteer or event spectator (Members with a criminal history of violence or with a legal restraining order against them by another member may be barred from membership and participation in all club activities);
· Never use abusive or vulgar language, or make racial, ethnic or gender-related slurs or derogatory comments at club events;
· Never make unwanted sexual or physical contact with other members (Members found to be listed on a sex offender registry, convicted of a sex crime, or caught having, creating, or distributing child pornography will be immediately barred from membership and all participation in club activities.);
· Always report violations of the Member Code of Conduct policy to the Admins in writing. SecureFee™ Registration Cancellation Insurance is offered in most states. If purchased, your registration may be covered for one of the covered reasons stated in the policy.
