2019 Covina Fun Run & Health Fair

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT ("AGREEMENT") EVENT: COVINA FUN RUN. (THE “EVENT”) FOR AND IN CONSIDERATION of being permitted to voluntarily participate in the "Covina Fun Run” on April 27, 2019, I, for myself, my personal representatives, assigns, heirs, and next of kin: 1. ACKNOWLEDGE, agree, and represent that my participation in the Event, as a participant in the 5K, 1 Mile, or as a volunteer or as a spectator, is voluntary and is independent of my employment (if employed by the Company and/or its affiliated entities). 2. ACKNOWLEDGE, and assume the risk of injury, and release, discharge and indemnify the Covina Rotary Club, City of Covina, Covina-Valley Unified School District, Charter Oak Unified School District, its volunteers and other associated with this event, from any and all claims for injury or damages arising from participation in the event. I know of no medical reason that would prevent my safe participation in this event. I hereby permit the Covina Rotary Club free use of my name and pictures in broadcasts, telecasts, periodicals, newspapers, and brochures in reports and promotion of this event.
