Thank you for registering to participate in this year's Bolton 5K!! To expedite the registration process and make the day way more ENJOYABLE for YOU and for our team, please complete the liability waiver below at your earliest convenience. NOTE: A separate form should be filled out for each person that you are registering. I know that participating in the Bolton 5K/Fun Run is a potentially hazardous activity and I should not enter and participate unless I am medically able to do so and properly trained. I acknowledge and assume any and all risks associated with running in this event including, but not limited to: falls, contact with other participants, the effects of weather, traffic, and course conditions. Knowing and appreciating these risks and in consideration of your acceptance of my entry, I hereby for myself, my heirs, representatives or anyone else claiming on my behalf, waive, release, and discharge Bolton Community Fund (BCF), Schartner Farm, the Town of Bolton, RaceWire and their agents assisting with the event, event volunteers and sponsors, and anyone else acting for or on behalf the Bolton 5K/FunRun from any and all claims of liability for death, personal injury, or damage of any kind arising out of my participation in this run. This Acknowledgement of Risk and Waiver of Liability extends to all claims of every kind whatsoever. I also consent to emergency treatment in the event of injury or illness. I grant full permission to BCF and/or any person or entity authorized by it to use my name, age, date of birth, finish place and finish time, as well as any photographs, recordings, or any other record of this event for any legitimate purpose. I understand that this event does not provide for refunds in the event of a cancellation, and by signing this waiver, I consent that I am not entitled to a refund if the event is cancelled before or during the event.
