I understand the potential hazards connected with running/walking and state that (participant�s name) _______________________________ is physically able to participate in the Road Race/Walk on Saturday, April 28, 2012, and has no known medical conditions which would indicate that he/she/I should not participate. As parent/legal guardian or myself, I waive any and all claims against any agency, sponsor, municipality or individual connected with the 2nd Annual School on Wheels of Massachusetts Race/Walk for any injury, loss, or damage arising from his/her/my participation in the Race/Walk. I hereby give my permission for his/her/my image to be used in any photo, film, or video of the event for any purpose. I agree to ensure that he/she/I will abide by the rules set forth by the organizers of the Race/Walk.
