I understand that dance is a physical activity and that there is an inherent risk of injury in participating in dance. I give approval for the enrolled student to participate in all aspects of Charles River Ballet Academy. I will not hold Charles River Ballet Academy or any of its teachers, students, or volunteers responsible for any injury to the enrolled student while participating in its classes, online classes, rehearsals, or performances. I also give approval for photos of the enrolled student to be used in CRBA ads, programs, poster, DVD cover, and web site. For online ballet classes, all attendees must make sure the area they will use is free of hazards before participating. I understand that I am enrolling in extra online ballet for Pre-Ballet May 11-June 13, 2020. SecureFee™ Registration Cancellation Insurance is offered in most states. If purchased, your registration may be covered for one of the covered reasons stated in the policy.
