Chelsea Chase 5K

In consideration of the services of Boys & Girls Clubs of Boston, Inc., the Jordan Boys & Girls Club, City of Chelsea, Chelsea Police, their agents, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf (herein collectively referred to as BGCB), I hereby agree to release, indemnify, and discharge BGCB on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows: The undersigned by registering in Chelsea Chase 5k: 1. I understand there are numerous risks and dangers inherent in participation in this activity, including but not limited to changing conditions, equipment, and a multitude of situations included in this release. Further, I understand that there may be damage, personal injury or death arising from my participation in these activities.2. I understand that as a participant I must follow the safety standards, guidelines and procedures established by the staff/instructor and that the staff of BGCB may deny me the opportunity to participate based on adherence to same.3. I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary and I elect to participate in spite of the risks.4. I give my consent to the instructors/staff of BGCB to arrange for treatment by medical personnel and agree to pay all costs associated with said treatment including transportation to a medical facility.5. I certify that I have adequate insurance to cover any injury or damage caused or suffered while participating, or else I agree to bear the costs of such injury and damage myself.* I further certify that I have no medical or physical conditions, which could interfere with my safety in this activity, or else I am willing to assume and bear the costs of all risks that may be directly or indirectly affected by any such condition.6. I acknowledge I am responsible for any damage incurred by me to any of the facilities and I understand that BGCB has the right to terminate the activities at any time. 7. I agree to release, forever discharge, indemnify, defend and hold harmless BGCB and its directors, overseers, officers, employees and agents from any and all claims, suits, costs, and attorney fees for damage and personal injury to me or my property resulting from the negligent acts or the consequences of their own negligence, that is, their failure to use reasonable care in any way.8. I consent to the use by BGCB and its sponsors of any pictures (video and print) for commercial purposes, or otherwise, of me in connection with the activities of BGCB.9. I understand that permission to use the BGCB facilities and/or participate in BGCB activities is being given to me in exchange for the execution of this liability release and covenant NOT to sue.10. I have read the above paragraphs and fully understand them. I understand that this is a Waiver of Liability / Assumption of Risk, which will legally prevent me or any other person from filing suit or making any other claims for damages in the event of personal injury, death, or property damage. I freely and voluntarily enter into this agreement. I have made no misrepresentation to Boys & Girls Clubs of Boston, Inc. SecureFee™ Registration Cancellation Insurance is offered in most states.
