The Runners High 5K

5K RACE PARTICIPANT WAIVER AND RELEASE FORM In consideration of being allowed to participate in the Runners High 5K race on 4/20/2024, I, the undersigned, acknowledge and agree as follows: Assumption of Risk: I understand that participating in the Race involves risks, known and unknown, including but not limited to, the risks associated with running, walking, or racing on roads, trails, or other surfaces, the risk of falls, the risk of injury or death due to vehicular traffic, weather conditions, and other potential hazards. I acknowledge that I am physically capable of participating in the Race and that I have no medical conditions that would prevent me from safely participating. Waiver and Release: I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge The Potters Recovery Center, its officers, directors, volunteers, sponsors, and all other individuals and entities associated with the Race from any and all claims, liabilities, or causes of action, whether known or unknown, arising out of my participation in the Race. This waiver and release extend to any and all claims for personal injury, property damage, or wrongful death that may arise from my participation in the Race. Medical Treatment: I understand that in the event of an injury or illness during the Race, medical treatment may be required. I authorize the Released Parties to seek and provide medical treatment as they deem necessary, and I accept responsibility for any costs associated with such treatment. Publicity: I grant permission to The Potters Recovery Center to use my name, likeness, and any photographs, videos, or other media taken during the Race for promotional and marketing purposes without compensation. Minors: If the participant is under 18 years of age, the undersigned parent or guardian acknowledges and agrees to all of the terms and conditions of this waiver on behalf of the minor participant. Agreement to Abide by Race Rules: I agree to abide by all Race rules and instructions provided by the Race organizers and officials. No Refunds: I understand and acknowledge that race registration fees are non-refundable. I have read this waiver and release form, understand its contents, and voluntarily sign it with the knowledge that I am releasing certain legal rights that I may otherwise have.
