Foley 5K Road Race

Please enter me in the Foley 5K Road Race, “Foley5K”, I agree to assume all responsibility for all risk of damage or injury to me as a participant in this event. In consideration of being accepted as an entrant in the Foley 5K, I hereby, for myself, my heirs, executors and administrators, release and discharge the City of Worcester, the Worcester Educational Development Foundation, Inc, the Central Mass Striders, sponsors, coordinating groups and any individuals associated with the race, their representatives and assigns, from all claims, damages, rights of action, present or future, whether the same be known, anticipated, or unanticipated, resulting from or arising out of, or incident to, my participation in this event. I hereby certify that I will not participate in the Foley 5K unless I am physically fit and sufficiently trained for competition. I also grant permission to any and all of the foregoing to use my likeness in all media. I grant permission for the use of my name and picture in any broadcast, recordings, photographs or any other account. I understand that bicycles, strollers, skateboards, roller skates or blades, animals, and headsets or earpiece of any kind are not allowed in the race and I will abide by these guidelines. If purchased, your registration may be covered for one of the covered reasons stated in the policy.
