ASSUMPTION OF RISK AND RELEASE OF LIABILITY A SEPARATE FORM IS REQUIRED FOR EACH PARTICIPANT In consideration of this entry being accepted, I, for myself, my heirs, executors, administrators, agents and assigns do hereby release and forever discharge the East Bay Coalition for the Homeless and East Bay Community Action Program, the Sponsors, the Race Committee, the Town of Bristol and State, the State of Rhode Island, and medical personnel, and any other individual, group of organization (volunteer or otherwise) who participates in organizing, managing or sponsoring the event, from any and all claims or liabilities arising out of my participation in the East Bay Coalition for Homeless 5K and Family Fun Walk. I hereby assume any and all risk associated with my participation in any of these events. I hereby certify that I have read and understand the above statement and agree to be legally bound by this agreement. I also attest that I am physically capable of competing in this event. This waiver of liability and assumption of risk is based on and hereby incorporates Rhode Island Statute Sec. 7-6-9 entitled EXEMPTION FROM LIABILITY.
