Accident Waiver and Release of Liability: Release of Name and Likeness I know that running/walking a road race is potentially hazardous activity, which could cause injury or death. I will not enter and participate unless I am medically able and properly trained, and by my signature, I certify that I am medically able to perform this event, and am in good health, and I am properly trained. I agree to abide by any decision of a race official relative to any aspect of my participation in this event, including the right of any official to deny or suspend my participation for any reason whatsoever. I hereby consent to receive medical attention which may be deemed advisable in the event of an injury, accident and/or illness during this event. I understand that all evacuation and medical costs for participants and volunteers will be borne by that person or their heirs. The race organizers and sponsors are in no way liable or responsible for medical costs or emergency evacuation. I attest that I have read the rules of the race and agree to abide by them. I assume all risks associated with running in this event, including but not limited to: falls, contact with other participants, volunteers, race officials, sponsors, walkers, baby stroller or “baby joggers”, the effects of the weather, including high heat and/or humidity, lack of hydration, traffic and the conditions of the road, all such risks being known and appreciated by me. I understand that bicycles, skateboards, roller skates or roller blades and animals are not allowed in either the 5K or the 1 Mile race. Baby strollers or “baby joggers” are permitted in the 1 Mile run/walk only. Having read this waiver and knowing these facts and in consideration of your accepting my entry, I hereby take action for myself, my heirs, executors, next of kin, administrators or anyone else who might make claim on my behalf, waive and release the Mission Possible 5K & 1 Mile Fun Run/Walk, the city of Tucson, Arizona, and Christ Community Church of Tucson, Inc., all event sponsors, their representatives and successors from all claims or liabilities of any kind arising out of my participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. This release and waiver extends to all claims of every kind or nature whatsoever, foreseen and unforeseen, known and unknown and I indemnify and hold harmless the entities or persons mentioned in this paragraph from any or all liabilities or claims made by any other individual or entities as a result of my actions during this event. I agree that Christ Community Church of Tucson, Inc. is not responsible for any personal property that is lost, damaged or stolen at the event. I understand that at this event and related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers, and assigns without compensating me in any way. Parent or Guardian waiver for minors (under 18-years old) if applicable. The undersigned parent or natural guardian or legal guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms as set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all parties referred to above from all liability, loss, cost, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to act and release said parties on behalf of the minor and the parents or legal guardian.
