SummerStage 5K Run/Walk at Lapham Peak

In consideration of acceptance of the entry, I the below signed intending to be legally bound for myself, my heirs, executors, and administrators, waive any and all rights and claims that I may have against SummerStage, the Lapham Peak Unit of the Southern Kettle Moraine State Forest and the WI DNR. This includes any sponsors, affiliates, agents, servants, volunteers, public employees, assignees, successors and their representatives for any and all injuries incurred to myself or my family in this event. I attest that I will participate in the footrace or walk event and that I am physically capable of completing such an event. Further, I grant full permission to any and all of the foregoing to use my name, likeness, and voice as well as any photographs, videotapes, and any other record of the event in which I/my family may appear, the reuse in any media and advertising and promotion for such broadcast and re-use.
