Let 's Move Day 5K/Running For Community

INFORMED CONSENT/RELEASE OF LIABILITY: I understand that participation in the Let’s Move Day 5K Fun Run presents certain risks and hazards, including but not limited to: muscle strains and sprains, bruises, broken limbs, dehydration, other serious medical problems or even death and other ordinary risks associated with strenuous physical activity. I acknowledge I do not have a past or present medical condition that may be affected by participating in this Activity or that I have obtained clearance from a physician before participating in this Activity. I voluntarily assume full and complete responsibility for any injury or accident, which may occur to me or my property during or in connection with this event. I hereby voluntarily Release, Waive, and Discharge The SDA Village Church, and their servants, agents, employees, participants, and volunteers assisting with this event (hereinafter “RELEASES”) from any and all liability, claims, demands, actions and causes of action whatsoever, whether such liability is based on negligence, arising out of or related to any loss, damage, or injury that may be sustained by me or to any property belonging to me while participating in the event. I further agree to Indemnify and Hold Harmless the Releases from any loss, liability, damage or costs, I may incur due to my participation in this Activity. PHOTOGRAPHY RELEASE I hereby grant permission to Let’s Move Day 5K Fun Run and its agents to photograph my image, likeness or depiction and/or that of my minor children (if applicable) and to edit, crop or retouch such photographs, and waive any right to inspect the final photographs. I hereby consent to and permit photographs of me and/or those of my minor children to be used Let’s Move Day 5K and its agents for any purpose, including advertisement purposes, and in any medium, including print and electronic. I understand that Let’s Move Day 5K Fun Run and its agents may use such photographs with or without associating names thereto. I further waive any claim for compensation of any kind for Let’s Move Day 5K Fun Run's or its agents' use or publication of photographs of me and/or those of my minor children (if applicable). I hereby fully and forever discharge and release Let’s Move Day 5K Fun Run and its agents from any claim for damages of any kind (including, but not limited to, invasion of privacy; defamation; false light or misappropriation of name, likeness or image) arising out of the use or publication of photographs of me and/or those of my minor children (if applicable) by Let’s Move Day 5K Fun Run or its agents, and covenant and agree not to sue or otherwise initiate legal proceedings against Let’s Move Day 5K Fun Run or its agents for such use or publication on my own behalf or on behalf of my minor children. All grants of permission and consent, and all covenants, agreements and understandings contained herein are irrevocable. SecureFee™ Registration Cancellation Insurance is offered in most states.
